SCHOOL REGISTRATION DATES:

Online Registration is now open. ALL STUDENT NEED TO BE REGISTERED ONLINE

In-Person Registration will be conducted on the following dates:

August 10, 2022 12:30 PM-3:00 PM AND 3:30PM-6:00 PM

August 11, 2022 8:00 AM-12:00 PM AND 12:30 PM-3:00 PM

August 12, 2022 8:00 AM-12:00 PM AND 12:30 PM-3:00 PM

August 15, 2022 8:30 AM-12: 00 PM

August 16, 2022 8:30 AM – 12:00 PM

TO EXPEDITE THE ENROLLMENT PROCEDURE, PLEASE SEE BELOW:

1. Please make sure your student has already been registered online

2. Please make sure you have ALL documents with you including: ▪ Birth Certificate ▪ Online Registration completed ▪ Parent Identification

THE FOLLOWING MUST BE DATED WITHIN THE LAST 60 DAYS:

▪ Immunization Record

▪ 2 Proofs of Residency (see list below)

▪ Required forms completed ▪ Registration Card (Hard Copy)

▪ Home Language Survey form

▪ Before/After School Supervision form

PROOF OF RESIDENCY ACCEPTABLE DOCUMENTS:

Documents showing Primary Residence:

▪ Deed/Mortgage Payment/Lease

▪ Legal Affidavit from Landlord or Religious Unit

Documents dated within last 60 days:

▪ Government letter from agency such as Assisted Housing, Food Stamps, Unemployment

▪ Payroll Stub

▪ Bank/Credit Card Statement

▪ Utility Bill

▪ Valid Utah Driver’s License/ID Card

▪ Valid Vehicle Registration

▪ Voter Registration Document Documents dated within Past 12 months:

▪ W-2 dated within past year ▪ Excise (Vehicle) Tax Bill dated within past year

▪ Property Tax Bill

UNACCEPTABLE DOCUMENT:

Power of Attorney Letters from friends/relatives PO Box Property Owned in boundaries

PARA ACELERAR EL PROCEDIMIENTO DE INSCRIPCION CONSULTE A CONTINUACION:

1. Asegurese de que su estudiante ya se haya registrado en linea.

2. Asegurese de tener TODOS los documentos con usted, incluidos:

• Acta de Nacimiento
• Registro en linea completado
• Identificacion del los padres

LO SIGUIENTE DEBE ESTAR FECHADO EN LOS ULTIMOS 60 DIAS:

Registro de vacunas

• 2 comprobantes de residencia (ver la lista a continuacion)
• Formularios requeridos completados
• Tarjeta de registro (copia impresa)
• Formulario de encuesta sobre el idioma del hogar (copia impresa)
• Formulario de supervision antes/despues del la excuela (copia impresa)

PRUEBA DE RESIDENCIA DOCUMENTOS ACEPTABLES:

Documentos que muestran la residencia principal:

• Escritura/Pago de Hipoteca/Arrendamiento
• Declaracion jurada delgal del arrendador o unidad religiosa

Documentos fechados dentro de los ultimos 60 dias:

• Carta del gobierno de una agencia como Vivienda asistida, cupones del alimentos, desempleo
• Comprobante de nomina
• Extracto bancario/tarjeta credito
• Factura de servicios publicos 
• Licencia de conducir/tarjeta de identificacion valida de Utah
• Matricula de Vehiculo Vigents
• Documento de registro de votantes

Documentos fechados dentro de los ultimos 12 meses:

• W-2 con fecha del ano pasado 
• Factura de impuestos especiales (vehiculos) con Fecha del ano
• Factura del Impuesto Predial

DOCUMENTO INACEPTABLE:

• Poder Notarial
• Cartas de amigas/familiares
• Apartado de correos
• Propiedad propia en linderos
